Prior to your upcoming appointment, please carefully read the following:

 

• You will be dilated at each visit. Expect to have blurred near vision and increased light sensitivity following your visit.

• If you are between the ages of 20-64, there most likely will be a fee for your examination. Please call or email us to inquire about our fees or to find out if you are eligible for OHIP coverage.

• Additional diagnostic tests are strongly recommended especially if you have personal/ family history of macular disease, optic nerve disease (ex. glaucoma), diabetes, or neurological concerns. Please note that some of these tests are not covered by OHIP and will result in an additional fee.

• If you require contact lens consulting and/or fitting, please inform a member of our staff upon scheduling your examination. These visits are not covered by OHIP.

• For children’s Myopia Control Visits, please note that myopia control tests are not covered by OHIP. Speak with a staff member to learn more.

• Cancellations and rescheduling must be received at least 48 hours before appointment time. Late notices will result in a cancellation fee.

• If you are driving, please park in the professional building’s gated parking lot for a flat $5 rate. Note, the spots closest to the building’s entrance are designated for patient parking only.
